Athletics

Before any student can step onto the field, court, or track for tryouts, all required medical and safety forms must be on file with the school nurse.

1. Concussion Information & Sign-Off Sheet: Completed and signed digitally during online registration.

2. Pre-Participation Physical History Form: Must be completed and signed by a parent/guardian prior to the exam.

3. Pre-Participation Physical Exam Form: Must be completed, signed, and dated by a licensed healthcare provider (physicals are valid for 395 days).

4. Pre-Participation Medical Eligibility Form: Signed by your healthcare provider confirming official athletic clearance.

Note: Medical forms can be uploaded directly during the online registration process. If you choose to submit them later, completed documents must be emailed directly to gms-nurse@golf67.net before tryouts begin.

All student-athletes must be registered through Campus Parent Portal prior to attending tryouts. Click the link below to start the registration process for the active season.

Parents of selected student-athletes will receive an email confirmation with instructions for completing final team enrollment.

Participation Fee

The athletic fee is $130 per sport. Fees are only due after a student makes the team and must be paid within one week of the final roster announcement.

Financial Assistance

Financial ability will never prevent a student from having the opportunity to participate. If your family is in need of assistance, please contact Dr. Norman directly.
